A:
  1. New unified business number: A different unified business number is treated as a different company or business. The new entity must establish an NHI-insured unit as a newly established company by submitting the NHI-Insured Unit Establishment Application Form (Form A), Insured Person Enrollment Application Form (Form D), company registration documents, and a photocopy of both sides of the responsible person's ID card, all affixed with the company and responsible person's seals. The application may be submitted in person or by mail to the NHI Administration.
  2. Cancellation of the old, unified business number: If the old Business Registration Certificate is no longer used, apply for its cancellation and provide proof of cancellation. Complete the NHI-Insured Unit Enrollment Termination Notification Form and Insured Person Enrollment Termination Application Form (Form L), affix the required seals, and mail them to the NHI Administration to cancel the unit.
  3. Simultaneous processing: The enrollment of all personnel under the new entity and the cancellation of the old unit may be processed at the same time.
